Life expectancy for 6 months or less, regardless of diagnosis according to the primary physician.

|
Palliative and comfort care are desired instead of curative treatment.
|
|
Patient and family desire physical, social, emotional and spiritual care from the hospice team.
|
|
Patient does not have to be home bound for hospice services.
|
